				Appeal for Denied Credits This form is for appealing the decision made for denied credits from the following Institution/Agency/Program only. Appeal forms should be filed as soon as possible after your admission to the university. Appeals must be written clearly and include supporting documents (all relevant information to verify and or support your appeal). If appropriate documents are missing, the appeal will be returned.
				Appeal for Transfer Credit Evaluation for Specific Courses This form is for appealing the transfer decision made regarding the courses on your General Education/Liberal Studies/MNTC worksheet only. Appeal forms should be filed as soon as possible after your admission to the university. All Transfer Appeals must be written clearly and include supporting documents (e.g. course description, syllabus or any other relevant information).

				Retroactive Drop/Registration Appeal This form allows a student to petition for a retroactive drop or withdrawal within 90 days from the end of a course due to extenuating circumstances.
